Low literacy and limited English proficiency are barriers to health and healthcare for many adults. This Health Information Toolkit is designed to provide health literate culturally and linguistically appropriate information on diabetes, high cholesterol, high blood pressure, and a balanced diet. Here is what’s included:
- Patient education videos are available in 12 languages including subtitled versions for those who are hard of hearing.
2. There is a downloadable pdf in each language with QR codes for each video. You can print these and keep in your clinics for quick reference or give to your patients.
3. There are downloadable medication instructions pdfs of commonly prescribed medications used to treat diabetes, high cholesterol, and high blood pressure written using health literate guidance. These are also available in all languages.
